How it works for families

  1. 1

    Hospital discharge

The hospital team decides it's time to leave — but being cleared to leave isn't the same as being ready to manage everything alone.

  2. 2

    Home with a maintenance plan

You leave with a clear plan for continuing progress independently, with the option to step down to outpatient visits first.

Common Questions

Transition care, answered plainly

Straight answers to what families usually ask before a hospital discharge or an inpatient stay.

What is transition care / transitional care?

Transition care — also called transitional care — is the structured support between a hospital discharge and being fully independent at home again. It covers both inpatient care (staying with us for closer supervision) and outpatient visits, depending on what your recovery stage needs.

My parent is being discharged after surgery — can they stay at Punarv?

Yes. Our in-patient program is built for exactly this situation — recovering from surgery with more supervision than a few weekly visits can provide. We start with an assessment to understand their specific condition and build a plan around it.

How long does inpatient rehabilitation usually take?

It varies by condition, so we don't quote a fixed timeline upfront. Your assessment sets a real baseline, and your length of stay is guided by progress against that baseline — not a fixed calendar.

Do you treat stroke patients?

Yes. Our Neuro and Spine Rehabilitation program is designed for stroke recovery and other neurological conditions, focused on regaining movement, balance, and independence through structured daily rehabilitation.

What should we bring for an inpatient stay?

Generally, comfortable clothing you can move easily in, personal toiletries, and a list of your current medications are a good starting point. We'll confirm the full details with you directly when your stay is scheduled.

Ready when you are

Not sure which service you need?

Our team can review the patient's condition, recent medical records, current ability, and recovery goals — and recommend the right level of care.
